1) If the pet is not already altered I agree to have the pet altered at the pre-paid cost to The Pet Connection...by ___________, or the pet will be returned immediately.

2) I agree to keep an identification tag attached to a properly fitted collar whichwill remain on the pet at all times, whether inside or outside of the house, and to obtain all of the necessary city licenses required by local authorities.

3) I agree to provide the  pet with necessary inoculations at the intervalsadvised by my veterinarian.

4) I agree to have the  pet under my control when it is not on the confinesof my property. A secured fenced area will be provided for dogs, including shelterfrom the elements. If the  pet is a cat, I agree to keep the cat as an indoor only pet. The pet will not be tied or chained.

5) If for any reason I cannot keep the pet, I agree to notify The Pet Connection of the availability of the pet and to return the  pet upon request.

6) I agree not to abuse or neglect the  pet and I authorize The Pet Connection at their board's discretion, to determine whether or not the pet has been abused or neglected.

7) I understand that any failure to perform the foregoing agreement will constitutea breach of contract. In the event of any such breach of contract, I authorize ThePet Connection to reclaim both possession and ownership of the  pet.

8) I understand that the pet covered by this document, is as far as can bedetermined by The Pet Connection, in good health and that they are is not  responsible for any medical fees incurred after this date.

9) I agree to give The Pet Connection a few visitation rights to ensure that the terms of this agreement are being observed.

10) While The Pet Connection may have provided assistance to an original guardian of the said pet, I understand that The Pet Connection has sole authority to enter into this contract. I hereby release and hold harmless The Pet Connection from any and all liability by reason of its acts, transactions and omissions with respect to the animal.
